Social Services Assistant - FT - Day

This full-time, onsite role assists the Social Services Coordinator in a non-profit healthcare facility. You will help plan and coordinate social services programs, counsel patients and families, manage documentation, and act as a liaison with community agencies. The position requires strong organizational and communication skills, with proficiency in Microsoft Office.

• Assist with social services program planning and coordination
• Counsel patients and families on emotional and mental limitations
• Complete social documentation and assist with grievance resolution
• Coordinate admissions, discharges, and community resources
• Liaise with social, health, and community agencies
• Proficient in Microsoft Office Suite
